In affected versions, vulnerability-lookup did not track or limit failed One-Time Password (OTP) attempts during Two-Factor Authentication (2FA) verification. An attacker who already knew or guessed a valid username and password could submit an arbitrary number of OTP codes without causing the account to be locked or generating any specific alert for administrators. This lack of rate-limiting and lockout on OTP failures significantly lowers the cost of online brute-force attacks against 2FA codes and increases the risk of successful account takeover, especially if OTP entropy is reduced (e.g. short numeric codes, user reuse, or predictable tokens). Additionally, administrators had no direct visibility into accounts experiencing repeated 2FA failures, making targeted attacks harder to detect and investigate. The patch introduces a persistent failed_otp_attempts counter on user accounts, locks the user after 5 invalid OTP submissions, resets the counter on successful verification, and surfaces failed 2FA attempts in the admin user list. This enforces an account lockout policy for OTP brute-force attempts and improves monitoring capabilities for suspicious 2FA activity.This issue affects Vulnerability-Lookup: before 2.18.0.

Some endpoints in vulnerability-lookup that modified application state (e.g. changing database entries, user data, configurations, or other privileged actions) may have been accessible via HTTP GET requests without requiring a CSRF token. This flaw leaves the application vulnerable to Cross-Site Request Forgery (CSRF) attacks: an attacker who tricks a logged-in user into visiting a malicious website could cause the user’s browser to issue GET requests that perform unintended state-changing operations in the context of their authenticated session. Because the server would treat these GET requests as valid (since no CSRF protection or POST method enforcement was in place), the attacker could exploit this to escalate privileges, change settings, or carry out other unauthorized actions without needing the user’s explicit consent or awareness.  The fix ensures that all state-changing endpoints now require HTTP POST requests and include a valid CSRF token. This enforces that state changes cannot be triggered by arbitrary cross-site GET requests. This issue affects Vulnerability-Lookup: before 2.18.0.

In affected versions, vulnerability-lookup handled user-controlled content in comments and bundles in an unsafe way, which could lead to stored Cross-Site Scripting (XSS). On the backend, the related_vulnerabilities field of bundles accepted arbitrary strings without format validation or proper sanitization. On the frontend, comment and bundle descriptions were converted from Markdown to HTML and then injected directly into the DOM using string templates and innerHTML. This combination allowed an attacker who could create or edit comments or bundles to store crafted HTML/JavaScript payloads which would later be rendered and executed in the browser of any user visiting the affected profile page (user.html).  This issue affects Vulnerability-Lookup: before 2.18.0.

Alchemy's Modular Account is a smart contract account that is compatible with ERC-4337 and ERC-6900. In versions on the 2.x branch prior to commit 5e6f540d249afcaeaf76ab95517d0359fde883b0, owners of Modular Accounts can grant session keys (scoped external keys) to external parties and would use the allowlist module to restrict which external contracts can be accessed by the session key. There is a bug in the allowlist module in that we don't check for the `executeUserOp` -> `execute` or `executeBatch` path, effectively allowing any session key to bypass any access control restrictions set on the session key. Session keys are able to access ERC20 and ERC721 token contracts amongst others, transferring all tokens from the account out andonfigure the permissions on external modules on session keys. They would be able to remove all restrictions set on themselves this way, or rotate the keys of other keys with higher privileges into keys that they control. Commit 5e6f540d249afcaeaf76ab95517d0359fde883b0 fixes this issue.

Auth0-PHP provides the PHP SDK for Auth0 Authentication and Management APIs. Starting in version 8.0.0-BETA1 and prior to version 8.14.0, session cookies of applications using the Auth0-PHP SDK configured with CookieStore have authentication tags that can be brute forced, which may result in unauthorized access. Certain pre-conditions are required to be vulnerable to this issue: Applications using the Auth0-PHP SDK, or the Auth0/symfony, Auth0/laravel-auth0, and Auth0/wordpress SDKs that rely on the Auth0-PHP SDK; and session storage configured with CookieStore. Upgrade Auth0/Auth0-PHP to v8.14.0 to receive a patch. As an additional precautionary measure, rotating cookie encryption keys is recommended. Note that once updated, any previous session cookies will be rejected.

Undici is an HTTP/1.1 client for Node.js. Prior to versions 5.29.0, 6.21.2, and 7.5.0, applications that use undici to implement a webhook-like system are vulnerable. If the attacker set up a server with an invalid certificate, and they can force the application to call the webhook repeatedly, then they can cause a memory leak. This has been patched in versions 5.29.0, 6.21.2, and 7.5.0. As a workaound, avoid calling a webhook repeatedly if the webhook fails.

Vyper is the Pythonic Programming Language for the Ethereum Virtual Machine. In versions up to and including 0.4.2rc1, `concat()` may skip evaluation of side effects when the length of an argument is zero. This is due to a fastpath in the implementation which skips evaluation of argument expressions when their length is zero. In practice, it would be very unusual in user code to construct zero-length bytestrings using an expression with side-effects, since zero-length bytestrings are typically constructed with the empty literal `b""`; the only way to construct an empty bytestring which has side effects would be with the ternary operator introduced in v0.3.8, e.g. `b"" if self.do_some_side_effect() else b""`. The fix is available in pull request 4644 and expected to be part of the 0.4.2 release. As a workaround, don't have side effects in expressions which construct zero-length bytestrings.

Vyper is the Pythonic Programming Language for the Ethereum Virtual Machine. In versions up to and including 0.4.2rc1, the `slice()` builtin can elide side effects when the output length is 0, and the source bytestring is a builtin (`msg.data` or `<address>.code`). The reason is that for these source locations, the check that `length >= 1` is skipped. The result is that a 0-length bytestring constructed with slice can be passed to `make_byte_array_copier`, which elides evaluation of its source argument when the max length is 0. The impact is that side effects in the `start` argument may be elided when the `length` argument is 0, e.g. `slice(msg.data, self.do_side_effect(), 0)`. The fix in pull request 4645 disallows any invocation of `slice()` with length 0, including for the ad hoc locations discussed in this advisory. The fix is expected to be part of version 0.4.2.

Spotipy is a Python library for the Spotify Web API. As of commit 4f5759dbfb4506c7b6280572a4db1aabc1ac778d, using `pull_request_target` on `.github/workflows/integration_tests.yml` followed by the checking out the head.sha of a forked PR can be exploited by attackers, since untrusted code can be executed having full access to secrets (from the base repo). By exploiting the vulnerability is possible to exfiltrate `GITHUB_TOKEN` and secrets `SPOTIPY_CLIENT_ID`, `SPOTIPY_CLIENT_SECRET`. In particular `GITHUB_TOKEN` which can be used to completely overtake the repo since the token has content write privileges. The `pull_request_target` in GitHub Actions is a major security concern—especially in public repositories—because it executes untrusted code from a PR, but with the context of the base repository, including access to its secrets. Commit 9dfb7177b8d7bb98a5a6014f8e6436812a47576f reverted the change that caused the issue.
